  • Personal Health Record with a Computer-Base

    A personal health record (PHR) is an initiative and medical records are managed by individuals. Applying a personal health record means to collect as much information as possible about your health past and present and their organization in a way that can easily verifiable and reproducible in terms of use may require. The starting point is to choose a personal health record that you type to enter information, and information and official documents such as x-ray reports, lab reports and electrocardiograms.
